• Pre-Enrollment Form - WISHING WELL PRESCHOOL - STEP #1

  Preenrollment for 2021-2022 school year begins January 4, 2021.

  This form, along with the registration fee, is simply used by us to gather your contact information, child’s name, age, requested days, etc. so that we can place your child in the appropriate classroom at Wishing Well Preschool’s North Royalton or Strongsville locations.

  Please note this form should be completed online and sent to us with a "click". Registration fees can also be paid online. Look on our Home Page for the Registration Fee button. We'll send a confirmation as soon as reasonably possible. Thanks

• Pre-Enrollment - ENRICHMENT CENTER - STEP #1

  This form, along with the registration fee, is simply used by us to gather your contact information, child’s name, age, requested days, etc. so that we can place your child in the appropriate classroom at The Enrichment Center.

  Please note: You can download this form, complete it and email directly to The Enrichment Center. However, this form tends to not work on mobile/Apple devices because it requires "Flash"....it does work fine on desktop computers with Windows and Flash.

• ENROLLMENT AND HEALTH INFORMATION - STEP #2 (JFS 01234)

  This is the enrollment form required by The State of Ohio to permanently enroll your child in any of our schools. Please note addition forms may be required if a special condition exists with your child. For instance, if a medical condition exists that requires a staff member to perform a procedure then the “Medical/Physical Care Plan” (form JFS 01236) is required. Or, if medication needs to be administered then form “Administration of Medication “ (form JFS 01217) is required for each medication to be administered.

• CHILD MEDICAL STATEMENT - STEP #3 (JFS01305)

  Physicians complete this form to verify childs immunizations records and general health.

• MEDICAL/PHYSICAL CARE PLAN (JFS 01236)

  This form is required if your child’s allergies, medical condition, special/modified diet, etc. requires a staff member to perform any specific care to your child.

• REQUEST FOR ADMINISTRATION OF MEDICATION (JFS 01217)

  This form is required if your child’s allergies, medical condition, special/modified diet, etc. requires a staff member to perform any specific care to your child.
